About DDD-PHMPS

The U.T Administration of DNH & DD has implemented the Public Health Management Platform application in all three districts of the U.T. This Comprehensive Primary Healthcare platform launched in 2023 comprises of a mobile interface and a web interface that is offline enabled for Facility health workers at the primary levels (SC/PHC/HWC) as well as other program functionaries of Vertical Health Programs at the UT as well as National Health Programs. The platform is configurable and extensible and implements all the workflows for the Community Health Workers for the RMNCHA program, the NCD Program, Nutrition program, the Anemia Mukt Bharat program, the Vector Born Disease program, etc and is an ABHA Complaint application.

The DDD PHMP application plays a pivotal role in managing Non-Communicable Diseases (NCDs) within the population, specifically targeting individuals aged 30 and above. This key program focuses on the prevention, early detection, and effective management of chronic diseases such as diabetes, hypertension, oral cancer, and more. By leveraging the DDD PHMP application, the aim is to improve health outcomes and reduce the burden of NCDs on the community.

Significance

A significant achievement of this program is the completion of the Community-Based Assessment Checklist (CBAC) forms for over 90% of the population aged 30 and above. Additionally, more than 80% of individuals in this age group have been screened for diabetes and hypertension through the application, demonstrating its widespread impact.

The DDD PHMP application is also seamlessly integrated with the national NCD portal, ensuring comprehensive data management and alignment with national health initiatives. Through these efforts, we are committed to enhancing the quality of care and promoting healthier lives for the population.
